The SIOG Surgical and Perioperative Optimisation Collaborative Interest Group was established to strengthen the role of surgery and perioperative care in the management of older adults with cancer.

As the global population ages, more patients present with complex needs that extend beyond standard surgical pathways. While surgery remains a cornerstone of cancer treatment, older adults are less likely to access surgical care or achieve optimal outcomes, often due to gaps in assessment, optimisation, and coordinated care.

The Collaborative was created to address this need by bringing together SIOG members across disciplines to advance a more integrated approach to surgical and perioperative care. It focuses on improving patient selection, optimisation, and recovery across the full perioperative pathway.

Aligned with SIOG’s strategy, the Collaborative promotes multidisciplinary collaboration, research, and education, supporting the integration of geriatric principles into surgical oncology and contributing to improved outcomes, functional preservation, and quality of life.

Focus

The Collaborative will focus on building an international network of clinicians and researchers working in surgical and perioperative care for older adults with cancer.

Planned activities include the development of educational content within SIOG, the organisation of sessions and initiatives at the SIOG Annual Conference, and the support of collaborative research and guideline development.

The group will also identify and share practical models of care that enable more older adults to safely undergo surgery, with a focus on improving recovery, preserving function, and maintaining independence.
